KRON4 obtained video showing the female driver of the Municipal Transportation Agency (MTA) train slumped over in her seat. The two-car train, filled with passengers, careened around a curve at an alarming speed, forcing a nearby car to swerve out of the way to avoid a collision. As the train accelerated to over 80 kilometers per hour, passengers were thrown from their seats, with many ending up on the floor. Screams of terror filled the air, with one woman desperately yelling, "I wanna get off!" and another crying out, "Oh my God!"

The sudden jolt and chaos eventually woke the driver, who scrambled to regain control and bring the runaway train to a halt. After stopping the train, she addressed the understandably terrified passengers, repeatedly urging them to "relax." "I'm sorry," she said, continuing with, "Relax, relax, relax. We didn't crash. Unfortunately, one female passenger suffered a concussion as a result of the incident, as reported by the San Francisco Chronicle. This highlights the very real potential for serious injuries when safety protocols fail.

An investigation was immediately launched following the incident, which occurred in the Sunset Tunnel near Duboce Park early on September 4th. The San Francisco Municipal Transportation Agency (SFMTA) announced on Monday that there were no mechanical issues with the train or the tracks. And this is the part most people miss... It wasn't a technical glitch; it was human error, plain and simple. The SFMTA confirmed the incident was "an error as a result of operator fatigue."

The SFMTA stated that the train "experienced a series of unexpected jolting movements while navigating a curve at an excessive speed of 50 mph (80 km/h)." They didn't specify the typical speed for that section of track, leaving some to wonder if this was a regular occurrence narrowly averted in the past. "Several passengers were jostled and fell due to the sudden motion,” the agency confirmed.

The agency has taken action, stating, “The agency is addressing the matter in accordance with internal protocols and the relevant contract, which included placing the operator on nondriving status.” This means the driver has been temporarily removed from operating trains while the investigation continues.

SFMTA director of transportation Julie Kirschbaum described the incident as “unacceptable,” and rightfully so. “We know this was a frightening experience for our riders, and we take that very seriously,” Kirschbaum said, emphasizing the agency's commitment to safety. "Safety is always our top priority. We are committed to accountability in response to this specific unacceptable incident and we are taking all necessary steps to keep Muni safe and reliable for all riders and the public.”
